For a screenshot of my profile page from my other trackers, does the application reviewer need my profile name? Should I obscure my profile name on the screenshot since it is published on a public hosting site?

Yes. The tracker's staff likely will have accounts on your trackers and will want to check if the stats are real. The only thing you want to obscure, when it applies, are stuff like PID or tracker announce url. If you're paranoid about it, just use imgbb.com, it allows you to set auto delete images after a certain time (choose at least 2 months, in my opinion).
