You need to take screenshots of your account stats on other private trackers (you're a member of).
Upload the screenshots in an image host service and paste the links in your application (that's the proof images part!)
Also, copy and paste (in your application) the URL of your profile page on those private trackers (you're a member of). That's the 'profile link' part!.
