MOST of us have probably been handed a survey or questionnaire at some point to provide feedback.

But one woman is applying that strategy to her love life, by sending out an ‘exit survey’ to her ex-boyfriends.

And not just long-term partners, but also to casual hook-ups.

The Sunreported that a photo of the woman’s survey has since gone viral after her friend posted a snap on Twitter.

She captioned it: “There’s this girl I know who sends an exit survey to every guy she casually AND seriously dates.”

And she seemingly outed her friend tweeting: “Give it up for @KMillertime9”.

Katie, from Washington DC, then retweeted her post.

Some of the questions include: “What is wrong with you?”, and “At what point did you know this wouldn’t work out?”

But she also cheekily asks if they can recommend anyone for her, seeing as it hasn’t worked out, and asks for numbers.

She signs off her survey by saying: “Please supply any additional feedback below.

“Katie can’t be alone forever!

People have praised how forward, and thorough, she is — and it’s been ‘liked’ over 20,000 times.

One person commented: “I have GOT to start doing this.”

Another said: “This is a work of genius.

“Finally dating has the structure and administrative bureaucracy it has always needed.”

A third added: “Everybody should do this.”
